Abstract
Les anastomoses lymphatico-veineuses n'ont pas montré leur intérêt dans les lymphœdèmes. Nous avons utilisé une nouvelle technique, celle de la transplantation ganglionnaire, qui consiste a prélever chez le même patient des ganglions et à les greffer par microchirurgie sur le membre présentant un lymphœdème. Patients et méthodes: Nous avons opéré 99 patients pour transplantation ganglionnaire: 58 transplantations chez des patientes présentant un lymphœdème des bras après mastectomie et radiothérapie pour cancer du sein et 41 patients pour lymphœdème congénital des membres inférieurs. Résultats: Les résultats montrent une diminution notable des infections de 80 % à 20 % et le périmètre des membres diminue considérablement pour revenir à une valeur diminuée de 50 % à six mois après l'intervention (80% de diminution des lymphœdèmes). Conclusion: Cette intervention représente une indication indéniable dans les lymphœdèmes en réduisant le volume des membres et le nombre des infections. Elle mérite d'être diffusée à plus grande échelle malgré sa difficulté technique, notamment dans les pays où l'éléphantiasis par filariose est endémique.
